If you are scheduling an appointment, be sure to leave your full name and insurance information if applicable, telephone number, and bring a copy of your insurance card to your visit. If you have vision coverage through Vision Service Plan, please also include the social security number of the person through whom coverage is provided as well as that of the person scheduling so that we may verify coverage for you and obtain the necessary paperwork to place any orders and submit for exam coverage.
